Release, Indemnification and Hold Harmless Agreement

In consideration of participating in all activities, entering the play area and/or attending a party at ONECHURCH PLAYLAND, the understanding, on their behalf, and on behalf of the participants named below, acknowledge, appreciate, understand, and agree to the following conditions:

I present that I am the parent or legal guardian of the participant(s) named below or have obtained permission from the parent/guardian of the participants to execute this agreement on their behalf. I agree that we are to comply with all stated and customary terms, posted safety signs, rules and verbal instructions as conditions for participation at OneChurch PLAYLAND. I acknowledge that activities that involve known and unanticipated risks which could result in property damage, physical, or emotional injury, paralysis or permanent disability or death. Risks included but not limited to broken bones, bruises, and or other bodily injuries caused by falls or contact with walls floors or other participants; medical conditions associated with participation in PLAYLAND and/or area. My participation in this activity is purely voluntary and I elect to participate despite all risks, real and potential. In addition, if at any time I believe that the event conditions are unsafe or that I or the participants named below, are unable to participate due to physical or medical conditions, then we will immediately discontinue participation. I, for myself and the participants named below, are of sufficient physical ability to participate in all OneChurch PLAYLAND activities.

I acknowledge that inflatables can be dangerous/ hazardous. Activities at ONECHURCH PLAYLAND are strenuous exercise which I and my child(ren) are physically fit and well to participate in.

I and my Child(ren) will comply with ALL rules and conditions, both posted and verbally requested by any ONECHURCH PLAYLAND ministry worker.

I will comply with any current health codes and restrictions at the time of participation to insure health and safety of myself and others.

Media Release: I understand and acknowledge as participants of ONECHURCH PLAYLAND, that images or audio/video recordings of myself and the participants named below may be used by ONECHURCH for all legal purposes, including posts to social media and other promotional platforms. This permission is required for participation in PLAYLAND activities due to the intentional online presence which ONECHURCH maintains.
